Northfield Community Partnership, Lifestyle Coach. Closing Date: on appointment of candidate.

Resilient, proactive and confident in developing and maintaining relationships with GP practices and healthcare professionals to generate referrals into the programme. The Lifestyle Coach will support individuals with type 2 diabetes to transform their health through personalised, locally delivered & culturally adapted support. The role will work directly with participants to enable them to achieve their goals. 

Salary: £30,000- £33,000 Annually
Hours: 34 hours per week
Contract: Until March 2027
Holiday: 33 days holiday - annually (including bank holidays), pro rata
Location: Office-based and across the Northfield district

Main Duties and Responsibilities

Key Responsibilities:

1. Manage triage of referrals, ensuring that only participants suited for the service are enrolled, and refer others to more suitable programmes as needed.
2. Complete baseline assessments with eligible participants to ascertain their starting point.
3. Work with eligible participants to create personalised action plans aimed at supporting them to achieve goals; covering weight reduction, physical activity and nutrition.
4. Help participants navigate various health services and local activities based on their needs. Where appropriate, introduce service users to community groups, activities and statutory services, ensuring they are comfortable. Follow up to ensure they are happy, able to engage, included and receiving good support.
5. Meet participants on a one-to-one basis at agreed intervals. Actively manage case load, maintaining regular contact to ensure participant goals are met and overall service outcomes are achieved.
6. Deliver health related workshops/group sessions as needed for the programme.
7. Outreaching for participants, actively engaging and connecting with individuals in the community, using targeted communication and partnerships to raise awareness and encourage their involvement
8. To understand and use CRM for all service users and updating as required. To ensure all data, monitoring and evaluation is kept up to date and reported back as required.
9. To adhere to data protection, safeguarding, confidentiality, information sharing protocols as required.
10. Participate in professional supervision, as required

Person Specification

Qualifications:

1. RSPH Level 3 Certificate in Health Coaching or equivalent experience and/or qualification (E)
2. Community, health, social care, counselling or other relevant experience and/or qualification (D)
3. English and Maths GCSE or equivalent (D)
Knowledge and Understanding 1. A good understanding of personalised care (E)
2. Understanding of evidence-based behaviour change principles and practice how a holistic approach to supporting service users leads to behaviour change (E)
3. Strong understanding of barriers to accessing health and local community services and how to navigate them (E)
4. A good understanding of ethnically diverse communities (E)
5. A proven understanding of safeguarding for children and vulnerable adults (E)

Experience and Skills:

1. Previous experience in health or community-related role supporting individuals to change their health or lifestyle (E)
2. Experience working collaboratively within multidisciplinary teams and a confident and approach to working with a variety of stakeholders (E)
3. Excellent written, verbal, listening and presentation skills (E)
4. Excellent IT skills and ability to do own administration using database and other IT packages (E)
5. Proven skills in collating information and data on community resources and organising it in up-to-date and accessible formats (E)
6. A commitment to equal opportunities and an understanding of the impact on individuals, families’ and communities’ health and deprivation (E)
7. Ability to implement relevant policies and procedures including safeguarding and Health & Safety (E)
8. Ability to speak more than one language (D)
