Project Coordinator - Carer Liaison

JOB BACKGROUND:

Forward Carers is an award-winning social enterprise, delivering services through a partnership of not-for-profit organisations in the West Midlands. Forward Carers offers a range of provision to unpaid carers ranging from carer assessments, advice and guidance; emergency back-up; carer training, wellbeing and much more. Forward Carers runs the Birmingham Carers Hub – a one-stop-shop for carer services and improving carer wellbeing. A key part of our success is developing and maintaining strategic relationships with key stakeholders in health and social care, including primary care colleagues. This will be critical for the role of Project Coordinator – Carer Liaison.  

JOB ROLE:           

This post will be responsible for the development and delivery of carer projects linked to community and primary care settings; as well as day to day tasks required for the successful delivery of Forward Carers services and projects. This will be achieved by:

  • Planning and coordination of Forward Carers projects; in particular, the Carer Liaison Project
  • Establishing and developing a good rapport with carers
  • Devising ways of monitoring and reporting on project progress and making improvements
  • Engaging with carers, partner organisations, health and social care providers and businesses to ensure effective project delivery
  • Establishing and maintaining relationships with primary care teams, social workers and community groups
  • Development of existing and new carer support groups ensuring carers become better informed about their caring role and support available to them
  • Working with colleagues to promote service take-up so more carers benefit from improved wellbeing including the new carer passport
  • Utilising appropriate systems and processes for recording service activity
  • Development and delivering key messages at carer groups and partnership events
  • Ensuring work is carried out to agreed standards and targets
  • Represent Forward Carers in a positive and professional manner at meetings and events
  • Undertake any training that may be necessary with the role
  • Take on other tasks and responsibilities in line with Forward Carers priorities that are appropriate to the level of the post

Education, Qualifications and Training 

  • Good standard of education to a minimum of GCSE (Mathematics at Grade C or above and English at Grade C or above), or equivalent (E)
  • Evidence of further learning and development (E)
  • Degree or professional qualification or management qualification or equivalent (D)
  • Evidence of further academic/personal development or equivalent (acquired through training and experience) (D)
  • Safeguarding training (D)

Experience 

  • Experience of working with family carers (E)
  • Experience of developing and maintaining key relationships with primary care and social work teams (E)
  • Experience of assisting or leading in the development of carer groups and services (E)
  • Experience of assisting or leading in the development of carer groups and services (E)
  • Experience of developing and delivering presentations to customers and/or stakeholders (E)
  • Experience of project and/or service development with comparable scope (D)
  • Experience of producing project reports(D)  
  • Experience of establishing or administering systems for monitoring and evaluation (D)
  • Experience of developing information for customers (D)

Knowledge or Awareness

  • Thorough understanding of the needs of carers and the types of issues they face (E)
