SIFA Fireside, Closing date: On Request

The Homeless Transition role is based at our main hub in Digbeth and sits within our Prevention services. This service provides support to vulnerable adults placed within exempt accommodation within the city and aims to facilitate a smooth transition into accommodation to ensure that they are settled and that it is of a suitable standard so that they are supported to live happy, healthy and independent lives within the community

This role works closely with other services at SIFA Fireside as well as the Birmingham City Council co-located Housing Team to prevent clients from re-presenting as homeless and is a crucial first step to a more settled way of living, intended to offer immediate support to people over a short period of time as they transition into accommodation and to support the drive to improve accommodation standards across the city.

Key tasks

1. Provide a person centred service to individuals taking a psychologically informed approach to advice and support ensuring that clients are provided with choice and control about how their support is provided.

2. Ensure that 95% of clients referred into the service have a first interaction within 24 hours of the referral being received.

3. Assist with the provision of practical support such as travel issues and address any immediate accommodation issues.

4. Use SIFA Fireside’s client management database to keep up to date, accurate notes about clients that are being supported.

5. Liaise with the accommodation provider as required to address any immediate issues relating to the accommodation or to the support in place.

6. Signpost the client appropriate ongoing services locally or citywide, to ensure they are able to access the support they need to stay safe and to start to live a healthier, happier life.

7. Build effective, positive relationships with clients, maintaining professional boundaries.

8. Provide opportunities for re-engagement with the service if needed by building trust and confidence with the client

9. Work alongside and in partnership with other advice hubs and Housing Options teams based at other sites across the city to process referrals quickly and efficiently.

10. Support the Senior Project Worker and Service Manager to monitor the take up and impact of service and the client journey, including case studies.

11. Work alongside volunteers to provide a safe, inclusive, effective and supportive service.

12. Ensure safeguarding issues are accurately identified and reported promptly in line with SIFA Fireside’s Safeguarding Vulnerable Adults policy.

13. Work in partnership with other agencies and organisations to ensure highest quality service is delivered to clients.

14. Keep up to date with relevant legislation and best practice.

15. Adhere to SIFA Fireside’s policies and procedures

16. Work at all times in alignment with SIFA Fireside’s values

17. Undertake designated duties within the service as directed by the Senior Project Worker and Service Manager.

18. Any other duties as reasonably required under the responsibilities of the post.

19. Adopt a flexible approach to working within a multi-disciplinary team.

Working conditions:

There is a requirement to travel locally to accommodation providers or outreach sites. Travel expenses will be paid for any journeys undertaken.

All frontline staff are required to work Bank Holidays on a rota basis.
