When patients are diagnosed with diabetes, they often lack the information they need to help them understand what the condition means for them. Community Liaison Volunteers will play a major role in changing this. They will receive referrals from local doctors and nurses when a patient is diagnosed with diabetes. They will provide much-needed information which will help patients understand and manage the condition.
