Birmingham is working hard to become a city where all citizens can thrive and achieve their potential. Over the past year, the City Council has been doing all it can to manage the challenges of the COVID-19 pandemic and maintain critical services that help to protect and support citizens.

We understand that it has been a difficult time for everyone and want to hear from you about the impact that COVID-19 and the lockdown measures have had on your health and wellbeing. Whether you're an individual, family, student, retired or newly arrived in the city, your opinion counts. We are keen to hear from people of all ages and in every community across Birmingham.

The survey will ask you about your background, your understanding and views regarding COVID-19 and the impact that lockdown measures have had on various aspects of your health and wellbeing, such as mental health, physical activity, employment and relationships. It should take about 15-30 minutes to complete.

We plan to publish the findings from the survey at a population rather than an individual level (for example in tables or graphs using summary results) and any quotes used from answers will remain anonymous.

While Birmingham City Council feel that the questions we have asked in this survey are anonymous and would not place you at risk of being identified as an individual, we will ensure that if any data is provided that creates such a risk we will process as per our privacy statement.
