The Transition Taskforce needs you!

Together for Short Lives is establishing a UK wide Transition Taskforce to lead the development of a coordinated strategic approach to provide care and support to young people with life limiting or life threatening conditions.

Led by David Strudley (CEO, Acorns Children’s Hospices) and working closely with partner organisations including Help the Hospices, Marie Curie Cancer Care, The National Council for Palliative Care and Preparing for Adulthood, as well as young people and parents/carers who have experience of transition, the taskforce is now looking to recruit expert individuals from a range of professional backgrounds and sectors.

Do you want to help....

  • lead the national drive to develop and integrate funded services for young people moving into adulthood?
  • stimulate new service developments and partnerships between adult and children’s service providers at national, regional and local levels?
  • provide guidance and support to commissioners of services for young adults with palliative care needs?
  • develop a UK-wide hub of information, training, research and good practice in relation to transition?
  • develop and implement evidence based best practice guidance and tools on transition?
  • engage with young people to ensure that services are appropriate to their need?
